There you can add info about your coin. Dont worry when you dont know something, referees can help you add these afterwards or they can send the request back to you.

You have to fill only few things (although more you do, the better) that is the name of the coin, then Issuing authority - this decides where request goes. And finally, you have to fill out source(s) of information at the bottom (sometimes writing just "personal observation" is enough) and click to send it.
