I think it is from Vatican City.
There is only one on here a bit like it (the reverse is similar) and I typed
PIETA in the Search part on here. This is not it though, and it is listed in the Tokens section ...

I believe the token you have is John Paul II, it doesn't appear to be on the Numista catalog yet - perhaps you could add it yourself?
