It could have just skipped a step of the production line maybe it fell off or was taken out for inspection and thrown in the wrong place?

For the coin: I checked also online lists for variations, and none of them lists this variation. I believe so that this one piece was destroyed somehow.

If it has same weight or even bit more than reported, it means that its an error, so very collectible and interesting piece.
