Mis-alignment 45 degrees

 Has anyone heard of any coin where one side is offcentre by 45 degrees? As in holding at top and bottom, then when one side is upright then flipping it around by the side, then the other side is not vertical. I know if both are vertical that is medal alignment, or the other side is inverted that is coin alignment.
 The link above is for a Bulgaria 10 Stotinki ( Medal alignment  ↑↑ ) which shows mintage for 1999 as 8,231,400 or 2000 as 10,000 Proof - but no mention of a known error for 1999 being mis-aligned.
 Not that I know much about how coins are made, but is there not some way that alignment for both sides is fixed, so that the dies cannot slip? I have seen forgeries of UK £1 that are off.
 Here are two photographs in front of a mirror - the one on the left is the 'error' one, and luckily I have a 'normal' one to compare with (on the right). Thanks :)

I have such coins. But, Hungarian only. The rotation may be small and large. Some examples. So you think?

That is called die axis, and in most medieval and ancient coinage, it goes around the clock.
Every angle of 360, really.
Hungarian MODERN coins with die axis diverting from 180 or 0
- I am sceptical...

 Update: There is now a 135 degree error to add, to the 45 degree that I have above. As in today I got in change (so did not buy) a UK ten pence that is wildly off axis. So 45 degrees plus another 90 degrees.

 

  Where has it been since it was made in 1992 I wonder - why has nobody spotted it before? The 1992 date was the first year of making these smaller (24.5mm) coins from the previous (28.5mm) size. If I had bought it I would have been a little suspicious, as it may have been made by a forger. It was only luck that I went into the shop that I got it (in pocket change) in the first place - the one down the road that I intended to go in had a notice on the door they were closed for two hours, so I walked (in the rain) to a different shop! I shall never sell it, as I collect errors - see other Topics below where there are other error coins ...
