Can anyone read German in old style font?

Hello,

I have this silver German medal/token. I don't know if I am reading the characters correctly, but google doesn't seem to recognize what I am typing in.

If anyone can tell me what it says, it would be greatly appreciated.

Not really pure fraktur script which makes it a bit harder also it uses old names and new names but not consistent.
It's the first mentioning of different places in THIS document.
