Old johann strauss coin/medal i need help to identyfi it

Looks like jewelry, is that a precious stone there? Ruby?  What does the other side look like?  Weight? Diameter?

From what i can see yes it is a gem size is about 20 mm and the weight 3.5-4.5 grams

I suspect yours was gold plated and the gem added and is probably one of a kind, essentially post-mint-damage.

I would guess significantly less that the one at auction mentioned above.  I don't think any collector of that type of medal would be interested in one that has been modified like that.  I could be wrong but I would be surprised.
