(en) The image is divided into three sections by stylized chains. The sections include a map of the New Hampshire coast with a three-masted ship at the top, a colonial style doorway and artist's initials on the right, and a church on the left. Where joined in the middle, the chains are superimposed by a strawberry (enameled red) with a ribbon displaying the dates 1623-1973. The image is surrounded by the words "PORTSMOUTH NEW HAMPSHIRE".

This was the official anniversary medal for the 350th anniversary of the City of Portsmouth, NH.
In addition to sterling silver (2000 minted), this medal was also produced in bronze (2500 minted), gold (2 minted), and an unknown number in platinum. This medal originally sold for US$12.
